SCHEME ELIGIBITY

Vehicle Segment : The scheme is applicable for all Jeeto load vehicles: Jeeto S6-16 (Diesel), Jeeto Plus Diesel, Jeeto Plus CNG, and Jeeto Plus Petrol
Eligible Customers : All Customers purchasing new vehicle during the scheme
Duration of the Scheme : Till July 31st 2021

INSURANCE SCHEME (INSURER- GO DIGIT INSURANCE)

Sum Insured : 1,00,000 per customer till age 60 years only
Policy Type : Indemnity Based
Scope of Coverage : All Covid related treatment expenses
Hospitalization cover : Yes, upto 1lakh sum insured
Home Care Coverage : Upto 10,000 within overall sum insured on production of proper bill and prescriptions, report
Waiting Period : 15 days from the date of coverage start date
Coverage : Self ( Customer only) no family cover
Policy Tenure : 1 year
Other Policy Conditions :
  • Comorbidity Waiver
  • No room rent or ICU room rent restrictions
  • Pre and Post hospitalization Cover for 30 days and 60 days respectively

POLICY & CLAIMS MANAGEMENT PROCESS

Policy Insurance

  • Data in the attached format required from M&M team on a mutually agreed frequency
  • A master Policy in the name of M&M Ltd ( or as per the name suggested) will be created and a deposit basis the number of expected customer with premium per head would be deposited in advance with the  Insurance company
  • All the information mentioned in the data are compulsory
    1. Vehicle ID : Serial number invoicing
    2. Delivery date : Vehicle delivery date
    3. Customer Name: Name of the customer as per invoice only
    4. Date of Birth : Correct date of birth in DD/MM/YY format
    5. Gender : Male/ Female
    6. Nominee : Nominee details or Legal Heir can be an option
    7. Nomine Relation: Relationship or Other
    8. Postal Code : Customer proper Postal Code
    9. Address : Customer Proper address ( should be less than 80 character)
    10. Mobile : Customer Mobile Number – used in Whatsapp
    11. Email : Customer email,address, else dealer email id ( ecard and policy)
    12. Pan Card : Customer Pan No
  • The cover will be from the date of invoicing for 365 days
  • An individual certificate will be issued for each customer under the Master policy
  • Customer will get a message from go digit team once his/her certificate is issued
  • Customer can go on the whatsapp number provided to download the policy certificate. Hence the customer whatsapp mobile number is important. Only register number can use this facility
  • Individual certificate will be issued within 17days from the data shared.

Step 1: Intimate the Claim to Insurance Company

  • Hospital name/ address
  • Diagnosis and treatment details
  • Policy number
  • Admission & discharge dates
  • Estimated/ Actual expense

Step 2A: Cashless Claim (TPA is MEDI Assist)

CASHLESS CLAIMS DOCUMENTS
  • COVID-19 Positive test report from ICMR authorized labs
  • Initial Consultation Papers
  • Govt valid ID proof
  • Doctor's advise for hospitalization

Please Note: Pre and Post Hospitalization expenses for cashless claim follow reimbursement claim process only if there is residual sum insured available for the insured. If insured already used 1lakh Sum Insured than policy will not pay any additional amount.

Step 2B: Reimbursement Claim-Document Upload/ Submission

BEFORE UPLOADING/ SUBMITTING ENSURE THAT ON ALL DOCUMENTS WRITE "FOR GO DIGIT COVID CLAIMS" IN YOUR OWN HANDWRITING.

  2. All other test reports- Blood & CT Scan etc.
  3. All Original bills (Consultation bills, Pharmacy and test bills, etc.) &
  4. Supporting Consultation Papers for all the Pharmacy purchased, or tests done
  5. Doctor’s advise for hospitalization
  6. Discharge Summary
  7. Govt valid ID proof (PAN, Voter ID, Driver's License, Passport)
  8. Bank details (Cancelled Cheque/ NEFT details)
HOME CARE TREATMENT CLAIM
  1. COVID-19 Positive test report from Govt/ICMR authorized labs
  2. All other test reports Blood & CT Scan etc.
  3. All Original bills (Consultation bills, Pharmacy and test bills, etc.)
  4. Supporting Consultation Papers for all the Pharmacy purchased, or tests done
  5. Supporting consultation papers with the progress, vitals monitoring, and the treatment given
  6. Govt valid ID proof (PAN, Voter ID, Driver's License, Passport)
  7. Bank details (Cancelled Cheque/ NEFT details)

Please Note: Insurance Company can ask additional documents other than mentioned above depending upon the case.

Step 3: Claim Status/ Settlement

Please Note: Insurance Company will settle the claim by transferring eligible payable amount in the bank account of the customer provided at the time of document submission. In case of additional document requirement, please upload the required document for settlement.
